Appointments and Scheduling
Appointments can be made between the hours of 8:30 am and 5:00 pm Monday through Friday. Please call 713.436.3488 to make your appointment. Most patients can get into the office the next day or possibly the same day for an appointment in one of our offices.

Monday - Friday 8:30 am - 5:00 pm

Please bring the following items with you to appointments:
  • Your insurance card or other health coverage information. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt.
  • X-rays or MRI's not older than twelve or eighteen months (if available).
  • Clinic notes or operative reports from your referring physician that relate to your condition.
Insurance Plans Accepted
We accept most major health care plans such as Aetna, United Health Care, BCBS, Cigna, Humana, HAS, HHPO, PHCS, First Health, Great West, Unicare and many more. please call 713.436.3488 for more information

Medical Records
If in the future you need to request certain medical records from our office, please be aware that the request could take ten days to two weeks before you receive the information. We will make every effort to fulfill all medical records requests as quickly as possible.

Billing Information
We accept Visa, Mastercard, Cash and Check. Co-pays are due upon arrival.

Surgery
If you require surgery, Dr. Burke will thoroughly explain the operative procedure, the recovery process and the type of rehabilitation program you will need. Our staff will pre-certify your procedure and discuss any financial implications with you.

After Hours Process
If you have a medical emergency please proceed to the nearest emergency room. However, if you need to contact a physician after hours, call the regular office number and our answering service will notify the physician on call to contact you.

Prescription refills will be filled between the hours of 9:00 AM and 4:00 PM Monday through Friday only.
